Oral Appliance Therapy (OAT)

Custom-titratable Mandibular Advancement Devices (MADs) are the standard oral appliance for treating Obstructive Sleep Apnea (OSA). They work by advancing the mandible and tongue forward, opening the posterior airway space during sleep.

Indications and Efficacy

OAT is indicated for:

  • Patients with mild to moderate OSA.
  • Patients with severe OSA who are intolerant of, or refuse, CPAP therapy.
  • Primary snoring without OSA (after PSG rule-out).

While CPAP is generally more efficacious at reducing the AHI to zero, MADs often demonstrate higher nightly compliance. Therefore, the "mean disease alleviation" (Efficacy × Compliance) is frequently comparable between the two therapies for mild/moderate cases.

Contraindications

OAT is contraindicated in patients with severe active periodontal disease, insufficient dentition to support the device, or severe temporomandibular joint (TMJ) pathology.
