The MSLT Protocol

The Multiple Sleep Latency Test (MSLT) provides an objective measure of the physiological tendency to fall asleep during normal waking hours. It is the gold standard test for diagnosing central disorders of hypersomnolence, specifically Narcolepsy (Types 1 & 2) and Idiopathic Hypersomnia.

Standardized Procedure

  1. Prior Nocturnal PSG: The MSLT must be immediately preceded by an in-lab nocturnal polysomnogram (PSG). This ensures the patient had adequate opportunity for sleep (typically requiring ≥ 6 hours of TST) and rules out other causes of sleepiness (e.g., severe OSA or PLMD).
  2. Nap Opportunities: The test consists of four or five 20-minute nap opportunities spaced exactly 2 hours apart, beginning 1.5 to 3 hours after waking from the nocturnal PSG.
  3. Testing Conditions: The patient lies in bed in a dark, quiet room and is instructed to "close your eyes, lie still, and try to fall asleep."

Scoring and Interpretation

The MSLT measures two key metrics:

  • Mean Sleep Latency (MSL): The average time it takes the patient to fall asleep across all naps. An MSL ≤ 8 minutes indicates pathological sleepiness.
  • Sleep Onset REM Periods (SOREMPs): If the patient enters REM sleep within 15 minutes of sleep onset during a nap, it is counted as a SOREMP. Two or more SOREMPs (with an MSL ≤ 8 min) are diagnostic for Narcolepsy.
